Fixed Whole-Blood Spectral Cytometry: Validate the Subset, Not Just the Storage Claim
A 2026 spectral panel study supports fixed whole blood for immune profiling. The practical question is whether each target population and epitope remains interpretable in the local workflow.
Preservation is part of the assay
Banking a specimen creates a promise: a later measurement will still answer the biological question that motivated collection. That promise must be defined population by population. A convenient shipping or storage workflow is useful only when the eventual result has a clear measurand, a traceable denominator and an acceptance rule that was set before reviewing the study outcome.
What the primary study contributes
Parthasarathy and colleagues developed 36-marker lymphoid and 35-marker myeloid spectral panels. Their comparative work examined fixed whole blood, fresh lysed blood and cryopreserved PBMCs. The long-term control came from one donor across 13 batches. Eosinophil recovery and selected subsets differed between workflows; the paper also cautions that fixation can affect epitope accessibility. Read the 2026 original article in Cytometry Part A.
The following framework is CFCM's practical interpretation, rather than a tested extension of that study.

Start with the cell population you cannot afford to lose
Before selecting a preservation route, write the intended result as a complete sentence: the frequency of a named subset among a named parent population, or the absolute concentration of that subset in the original specimen. These are different outputs. A frequency can remain stable when numerator and denominator fall together; it can also change when the target survives but another component of the denominator is depleted. A visually convincing gate alone cannot settle which explanation applies.
Build a local comparison around the difficult compartment, rather than averaging it into a large lymphocyte group. Include samples close to the proposed reporting boundary and keep failures visible. Record collection-to-processing intervals, specimen matrix, storage history, thawing steps and the precise antibody configuration. If a specimen has no interpretable target events, report that condition separately from a measured absence.
Separate recovery, signal resolution and interpretation
CFCM recommends three distinct questions for transfer studies. First, are enough target cells recovered for the intended analysis? Second, does the staining still separate the populations on which the gate depends? Third, does the final result agree closely enough to support the intended use? Passing one question does not answer the others. A preserved major-lineage proportion does not establish reliable classification of a dim, small or treatment-altered subset.
A useful worksheet links every critical marker to its role in the gating hierarchy, the relevant positive and negative controls, an observed failure mode and a planned response. For an ambiguous boundary, retain representative paired plots and explain why an operator chose a gate. Do not silently move boundaries until the paired frequencies match. Any revised clone or fluorochrome combination should enter the same change-control record as the preservation change.
Read storage claims with the right denominator
A laboratory considering long storage should distinguish repeated measurements of a control from evidence across independent people and disease states. Repeated aliquots can be valuable for monitoring analytical drift, but the number of batches must not be presented as the number of biological donors. Define what evidence is needed for your own storage interval, target population and clinical context before adopting an expiry rule.
For adoption, CFCM would ask for a prespecified agreement assessment and an explicit treatment of non-evaluable specimens. A non-significant difference is not an equivalence test. Choose allowable differences according to the consequences of misclassification, and examine individual paired results rather than relying only on an overall association.

Preserving a sample and preserving a measurement are different validation targets. Name the cell, marker, denominator and decision before deciding whether stability is adequate.
